Patient's Rights
Every patient has the right to:
- Receive respectful and compassionate care in a safe environment
- Be treated with dignity and without discrimination
- Receive information about their condition and treatment options
- Make informed decisions about their medical care
- Refuse treatment to the extent permitted by law
- Privacy and confidentiality of medical information
- File a complaint or grievance without retaliation
- Access their medical records
Patient's Responsibilities
Every patient is responsible for:
- Providing accurate information about their medical history
- Following treatment instructions and medical advice
- Paying for services in a timely manner
- Treating staff and other patients with respect
- Notifying staff of any changes in their condition
Respect for Property and Person
Patients have the right to safe, respectful treatment free from physical or mental abuse, harassment, or threats. The facility maintains a zero-tolerance policy for any form of violence or threatening behavior.
Advance Directives
Patients have the right to make decisions regarding their medical care through advance directives. The facility will respect the patient’s wishes as documented in valid advance directives in accordance with Florida law.
Complaints and Grievances
Patients have the right to file complaints or grievances regarding their care. Complaints should be directed to the Business Office Manager at (813) 995-8982. Patients may also file complaints with the Florida Department of Health at (850) 245-4339.
Physician Financial Interest and Ownership
The following physicians have a financial interest in or ownership of Surgery Center of Florida:

In accordance with Florida law, patients are hereby notified of this ownership interest.
